Please arrive early to your appointment—the office needs time to handle your paperwork. Please bring the following to your appointment:

• Your insurance card
• Physician referral forms if required by insurance
• Any recent test results or appropriate records
• Completed patient forms OR:
  • A list of current prescriptions and/or over-the-counter medications you are taking, including dose and frequency
  • Pertinent information about your medical and surgical history
